Position Description

Title: Licensed Practical Nurse

Location: Middlebury, Vermont

Job Type: 16 Weeks Contract

Shifts: 12-Hour Night Shift, rotating as needed, with occasional floating required.

Job Summary:

The LPN will provide nursing care to long-term care residents. This role is ideal for professionals with experience in long-term care, particularly in dementia care, and those who are comfortable working with a diverse patient population aged 60 and older.

Key Responsibilities:

  • Provide nursing care to long-term care patients in a skilled nursing facility.
  • Assist in the daily care and comfort of residents, including administering medications, dressing changes, and other treatments.
  • Monitor vital signs and report any changes in patient condition to the nursing team.
  • Assist with patient mobility and daily activities.
  • Maintain accurate patient records using the EPIC documentation system.
  • Participate in care planning and patient assessments.
  • Ensure a safe and comfortable environment for patients.
  • Float between units as needed, including Memory Care and PAC units.
  • Follow all infection control and safety protocols.
  • Ensure compliance with all regulatory and facility policies.
  • Provide compassionate care to patients and assist with family communication.

Qualifications:

  • License: Must be a Licensed Practical Nurse (LPN) in the state of Vermont.
  • Experience: A minimum of 1 year of clinical experience, particularly in long-term care, is required.
  • Certifications: Dementia-Capable Care certification is preferred but not required.
  • Skills: Proficiency in using EPIC for documentation and medication administration.
  • Other Requirements: Ability to work independently and as part of a team in a fast-paced environment.

*Offered pay rate will be based on education, experience, and healthcare credentials.
